目的:探讨膳食炎症指数(Dietary inflammation index,DII)与心血管-肾脏-代谢(Cardiovascular-kidney-metabolic,CKM)综合征的关联性。方法:本研究使用美国国家健康和营养检查调查(National Health and Nutrition Examination Survey, NHANES)1999—2020年的数据。根据CKM综合征的临床严重程度,将研究对象的CKM患病情况分为4个分期。DII通过膳食回顾法计算。采用单因素分析(连续变量的组间比较选择方差分析,分类变量的组间比较选择卡方检验)和无序多分类Logistic回归模型探讨DII与CKM综合征的关联,并通过趋势性检验(Ptrend)分析DII四分位分组与CKM综合征分期之间的线性趋势。结果:最终分析包括16 530例研究对象,基线时CKM综合征分期涵盖0~4期。总体DII评分为(1.40±1.85)分,且DII四分位分组随着CKM综合征分期增加而显著升高,呈线性增加的趋势。无序多分类Logistic回归模型分析结果表明,在调整混杂因素后,更高的DII与CKM综合征发生风险仍然相关。在完全调整模型中,CKM4分期的最高DII组(Q4)风险最为突出。结论:DII评分的升高可能通过促进系统性炎症这一潜在机制影响CKM综合征的发生与发展。应重视对高膳食炎症负荷的早期识别与干预,推广抗炎饮食模式,从而有效预防和管理CKM综合征。
Abstract:Objective : To investigate the association between the dietary inflammation index(DII) and cardiovascular-kidney-metabolic(CKM) syndrome. Methods : The data from the National Health and Nutrition Examination Survey(NHANES) from 1999 to 2020 was used. Participants were classified into four stages based on the clinical severity of CKM syndrome. The DII was calculated using dietary recall data. Univariate analysis(analysis of variance for continuous variables and chi-square test for categorical variables) and multinomial logistic regression were employed to examine the association between DII and CKM syndrome. Additionally, a trend test(Ptrend) was conducted to assess the linear trend across DII quartiles in relation to CKM syndrome stages. Results : The final analysis included 16 530 participants, with CKM syndrome stages ranging from 0 to 4 at baseline. The overall mean DII score was 1. 40±1. 85, and DII quartiles showed a significant increasing trend with advancing CKM syndrome stages. The multinomial logistic regression analysis indicated that, after adjusting for confounders, higher DII scores remained associated with an increased risk of CKM syndrome. In the fully adjusted model, the highest DII quartile(Q4) demonstrated the most pronounced risk for CKM stage 4. Conclusion : Elevated DII scores may influence the onset and progression of CKM syndrome through the potential mechanism of promoting systemic inflammation. Early identification and intervention targeting high dietary inflammatory load, along with the promotion of anti-inflammatory dietary patterns, should be emphasized for the effective prevention and management of CKM syndrome.
